Add New Database Problem - ISPConfig 3

i add new database and the result in control panel show it to me active, only the database missing in mysql... please what go wrong?

Most likely the mysql root password in the file /usr/local/ispconfig/server/lib/mysql_clientdb.conf is not correct.
