Amavisd has 3 levels:
1) Spam tag level: This is the level where information about the spam status is added to the header. You can set this to a low level like 0, as no actions are bound to that level.
2) Spam tag level 2: This is the level for tagging a email as spam. So thi score is the relevant score for moving spam to the junk folder.
3) Spam kill level. As the name says, spam with a score above that level will get deleted.
How do i use the "Addr. extension SPAM" to move spam to a specific folder? Like "Skräppost" Swedish name for Junkmail. To i type skräppost or +skräppost?

The "Addr. extension SPAM" changes the address of a email so that you can filter it in the lda, it does not move spam mails to a folder automatically.
You can use the email filter functions in the mailbox settings to move spam to a maildir folder of your account based on the title or address of the email.

Is not possible just move the spam mail to junk folder without modifying the subject?
Yes, thats possible by just enabling the "move to junk" checkbox in the mailbox settings in ispconfig. But the first poster wanted to move junk to a custom folder and not the system junk folder. For a custom folder, you have to create a custom rule and the easiest way to do this is to rely on the title.
