Default Apache Mod_Proxy Problem

Hi All,

I'm having problem getting apache mod_proxy to work. I'm getting this error and not sure why and how to fix.

Code:
[Wed May 21 21:16:45 2008] [error] [client 192.168.1.5] proxy: Max-Forwards has reached zero - proxy loop? returned by /
I'd appreciate any comments/suggestion to help me fix this problem.

Note: To setup my proxy i've pretty much followed this example in this how-to:
http://howtoforge.com/apache_reverse_proxy_ispconfig

My setup is on debian etch 2.6.18-1...

Thanks in advance for anyone assistance.
