It turns out that the updated version of mySQL added the flag "skip-bdb" to the mysql config file in /etc/mysql/my.cnf

removed name from users file

The name was located in:

/home/admispconfig/ispconfig/users

I have now removed it, and am able to create the user using ISPConfig.
