It is caused by AddHandler in Apache so we switch to SetHandler in combination with FilesMatch.

Take a look at "iotop", "iftop", "netstat -ntap" while such load peaks occur.
Maybe you find something special in there.
You may have to install iotop and iftop as they are not installed by default.
