After downloading "http://www.ispconfig.org/downloads/phpmyadmin-2.9.1.pkg" and installing it following the instruction... whenever I try to log in it is asking me for a password. I used the ISPConfig password as well as the root password of my server and none seems to work. Is there anything else I need to do?

You only can login with mysql user accounts not with your ISPConfig administrator account or webusers.

The account with the highest level is the mysql root account as defined during the "ISPConfig Perfect setup".
