Adding MySQL dB

Can someone enlighten me as to why I am, from time-to-time, unable to add MySQL dB to a site? I get an error that says "You cannot add a database to this site". No explanation ... nothing. I thought I had a clue when I was able to add a dB on some sites AFTER I had added a user but I am trying now with a new internal admin site and I cannot seem to add a dB. This happens more often than not when I set up a new site.

I am on CentOS5, MySQL 5.x

Please make sure that the checkbox to add databases is checked in the website and that the max. database limit of the website and the reseller is not reached.
